Request Medical Records
Texas NeuroRehab Center takes the protection of your personal health information seriously. Our release of information process ensures that you have access to medical records in a timely manner without sacrificing privacy.
The Health Information Management Department is open Monday through Friday from 9 am to 4 pm, excluding holidays, and can be reached by calling 512-462-6616 for any questions you may have regarding the options below or any general release of information questions.
For Patients and Their Representatives
Patient Portal
Available for medical patients only (Bluebonnet, Sage, Willow, and Magnolia Units); not available for behavioral patients currently.
Health Records Online consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day. Some medical records may only be available through the Health Information Management Department.

Medical Records Request
You may also request a copy of your medical records through the online portal. If requesting as legal guardian/guardian ad litem/POA a copy of the legal papers must be included with the request. You may incur a cost for the copying of records.

For Healthcare Providers
Please fax your written request on letterhead to us at 512-462-6771. If you are unable to fax a request, you may request records through the online portal. To request a copy of your medical records through the online portal, click on the link below and follow the prompts for online medical record request submission.
For Insurance, Attorney, Disability and Third-Party Requests
